Dear Tom,
Earlier, I sent an indirect link to the AMS Style Guide.
I've now been reminded of the direct link:

This guide is based on current practice, based on Ellen Swanson's
Math into Type, which is ultimately based on Chaundy, as quoted
by Peter Wilson.

The Style Guide does not spend spend as much time on bibliographic
references as Swanson since it's now assumed that most authors
will use something like BibTeX, and will pick up most bib data
from MathSciNet.
