[texhax] Blank first page problem (how to remove?)

So it seems notepad in Windows have done some formatting of the file
formatting which I didn't notice. At least I've learned some lessons,
and in particular learning to use the log-file produced for error
tracing. 

Sorry about the noise and thanks to all for pointing me in the
right directions to resolve this!
