[texhax] rejected bibtex entry

Can anyone see why makeindex would reject the following?
	\index{Euclidean n space at Euclidean $n$-space}
I am getting a "1 rejected" message on the console when I run  
makeindex in TeXShop, and this particular entry does not print.
