I use a .tex file.  I just released it it for you at:

http://www.ccrnp.ncifcrf.gov/~toms/workcard.tex

It makes a set of cards on a page.  You have to cut them with a paper
cutter but they work quite well.

It does a picture multiput and it calls a 'guts.tex' file for you to
insert whatever you want.
