[tex4ht] Three questions about errors when using mk4ht to create MathJax compatible files

    After the upgrade I no longer had mzlatex as a script.  

This is the only part I can answer ... yes, sorry it disappeared.  I
plan to restore mzhlatex, xhlatex, and oolatex (and any others people
report) in a future update.  I was a little over-exuberant in trimming
all the wrapper scripts a couple years ago.
