I think, there is some fundamental problem with your tex4ht
installation. I tried a file called, test.tex, I could not replicate
your problem. The log in my case for the corresponding location looked
like:

Anyway, I am attaching an archive (test.zip) of all files of my test
run which might be of help to you. You may kindly do the same and send
