I'm don't know.  Is the LuaTeX solution written up by Luigi in the
article you pointed to (http://www.ntg.nl/maps/40/07.pdf) working well?

All I can say is it clearly it would be good if luatex and xetex did the
same thing, if possible.  XeTeX people, dvipdfmx people, thoughts please?

I find it sad that standards interpretation is being driven by a poor
implementation decision in Acrobat, but such is life, I guess.  (At
least, that's how I read Jonathan's msg that you also pointed to,
