I've tried to reach Paul Pichaureau, the mathdesign developer, on problems with the mathdesign mdfga support files but I've had no response. 

It turn out that it is not possible to use mdfga in latex+dvips+ps2pdf mode because the PostScript names of the fonts distributed in mdfga.map are incorrect. The fix is to make the following replacements.

FSGaramondSmallCaps-Bold    ==> FSGaramondSmCaps-Bold
FSGaramond-Regular          ==> FSGaramond
FSGaramondSmallCaps-Regular ==> FSGaramondSmCaps
FSGaramondExpert-Regular    ==> FSGaramondExpert

so that the contents of mdfga.map reads, after correction,
