[tex-live] tlmgr returns 0 even on failure

Hi Clea,

    tlmgr update --all ...
    Exits with 0 even though parts of the update process failed. In
    particular, not all formats were successfully regenerated.

I think I've made changes so that tlmgr will exit 1 when any of the
various subprograms it calls at the end (fmtutil, updmap, mktexlsr)
fail.
