[tex-live] Pre-Final CD/DVD cover

Hi,

with a lot of support from Karl to get the English texts right,
I made a pre-final version of the cover and labels.
I hope that I will just have to add the French and Polish questions
and answers where the German and English Q&As now appear twice, when
Daniel and Staszek are ready with the translations, and to correct
the typos you might find.
