[tex-live] Technical showstoppers for TL2003

The most simple solution up to now is to add 2 lines in texmfmp.c to say
that if no .tcx file has been specified, then we use cp8bit.tcx
(possibly renamed to default.tcx). And yes, we need to recompile
everything in texk/web2c/.
