ptmr7t -> ptmr8r is done by virtual fonts, not by \usepackage[...]{fontenc}

No. These fonts are included in teTeX's texmf tree which Gerben uses.
He does not need to install additional font packages.

No, teTeX includes the urw fonts with "Berry" names.
