[texhax] Configure page and header in style book

Hi,
try without amsmath to see if the problem goes away:
I recall I had the same problem some years ago involving an interaction
between babel and amsmath. This was reported as a bug, and I think it
was fixed later (sorry for not being more precise I would have to dig
into my mail archives).

then update amsmath and babel (and the spanish.ldf or whatever it is called
file for spanish with babel) to see if the problem does not go away

I which case I would suggest you have a look at the fancyhdr package.  
It gives very good control over headers and footers.

Just one thing. What happens when you use the default page layout  
values? You have specified various page parameters which may be  
causing the alignment of header elements to shift.
