[texhax] expansion in makeindex

I'm confused about the following kinds of phenomena with MakeIndex:

1.When I have in my text file an entry
	\index{distance! in \mathbb{R}},
it prints in the idx file as
	\indexentry{distance!in @\@mathbb {R}}{1}
and then in the printed index prints as
	mathbb{R}

2.What am I supposed to do to get it to print as a blackboard-bold  
R?  Furthermore, what about
user-defined macros (for example, I have one
	\Realsto[1]
which expands as
	\ensuremath{\mathbb{R}^{#1}}
---how can I use it in an index-generating command

3.(for example, I have a macro
	\deffontcustom[2]
which expands as
	\textbf{#1}\index{#2}
---do I do something different when entering an entry involving a  
macro like the one above?
