The documentation seems clear to me now.  But in the course of checking 
this, I noticed the following glitch:  Using tlmgr r14573 with a 
completely up-to-date installation from the Utah mirror, I get the 
following:

$ tlmgr update --self --all
tlmgr: installation location ftp://ftp.math.utah.edu/texlive/tlnet
tlmgr update: please specify a list of packages or --all.
Restarting tlmgr to complete update ...
tlmgr: installation location ftp://ftp.math.utah.edu/texlive/tlnet

So tlmgr claimed incorrectly that I didn't specify a list of packages, 
and it also restarted unnecessarily (given that nothing needed to be 
updated).
