Many of those patches are for packages that aren't maintained in TeX Live.
At a glance: afm2pl cjkutils detex(?) dvi2tty dvipos gsftopk lacheck
musixflx ps2pkm pdftexdir xdvik ps2eps psutils.

I hope you'll be able to send them to the authors.

As for the ones that are handled in TL, I will take a look as soon as I
have a chance.  Adding $DESTDIR in the various Makefile's seems
straightforward enough ...
