I just posted this answer on ctt ...

tex4ht is not an easy thing to configure.  I don't know specifically
what to tell you, but we expend a lot of time getting it to work in TeX
Live.  I suggest looking at the tex4ht setup in the TL 05 distribution
(http://tug.org/texlive) -- or maybe just using TL'05 :)?

Failing that, I suggest writing Eitan, after looking through his web
pages.  Unfortunately I myself have never been able to get all the
details straight from the documentation, and end up asking him lots of
questions.  Gratefully, he is endlessly patient :).
